Hi Clare:
I sent out a similar message a few weeks ago and was able to get some great
examples. I took what I got and what we had and updated the permission
form. I am at a junior high (6-8) and we have a separate YA section. Any
students that want to check out books from the YA section have to have the
permission form signed and in file. Here is our copy, I hope you find it
useful.
