I do this.  We meet during lunch and call it BLT=Books, Lunch, and Talk.
We have three lunch hours, and I meet with the kids all three every week
while the kids eat their sack lunches.  I find if I do it just once a
month, the kids forget to come.  For book selection, I ask the kids for
suggestions on type of genre, and then I bring in several books that they
vote on.  When Rebecca Caudill nominees are announced, we read three of
those at least so all can vote.

Each week I write questions or use ones I find on line to hold discussion.
The kids can also bring up their own questions.

At the first of the year I had the kids come up with rules.  Here is what
they decided:
1.  The book we choose no one should have read before.  Therefore, when I
bring in books, the first thing we do is throw out the ones anyone in the
group has read.
2.  You can read ahead (we divide the book into about 100 pages per week),
but you can't be a spoiler when you come to the meeting.  You are only
allowed to discuss that week's readings.
3.  If you haven't read the pages for the week, you don't come.
4.  Let everyone have a chance to speak.  For my talkative sixth grade
group, we have a talking bookworm.  If you don't have the worm in your
hand, you can't talk.
