[ISLMA-Share] Librarian Evaluation Instruments

5%  of my 30% is MAP scores. The other 25% is a type III that I took from the ISLMA PERA Committee-note taking for 3rd graders. I do a pre and post test and take about 5 weeks (Teaching them once per week) to teach the skill.
