[ISLMA-Share] ISLMA Conference Evaluation

On behalf of the entire Conference Committee, thank you for attending the ISLMA 2016 Conference. We hope you found your time valuable and are able to LEAD new ideas to try at your school. Please complete the following 5 Question ISLMA Evaluation Form<http://bit.ly/2fwvCgt>. We will use this information in planning the 2017 and future ISLMA Conferences. Please complete the 5 Question ISLMA Evaluation Form before Wednesday, November 30, 2016.


Please also remember to submit an ISBE evaluation for each session you attended at conference. You can find the QR code and instructions on page 12 in the Conference program. You can also go to http://bit.ly/confeva16


You must submit the Evaluation form for EACH session in order to receive the Evidence of Completion (EOC) form. (This may take 7 - 10 days as ISLMA must verify attendance.)

Evaluations must be submitted by DECEMBER 20, 2016.
