I started a new system this year and we really like it. Here it goes...

Student take either a blue or yellow Book Notes bookmark when they check
out a book. A yellow is for a Caudill or Mock Sibert, a blue is for any
other book.

The bookmark requires them to take a few notes; things like important
characters, main conflict, their favorite event, etc.

When they finish their book and bookmark they bring it to the circ desk.
We sign off on the top of it after a quick looksie. This is also where we
can ask questions if we want to check for honesty. If it's yellow that's
my visual reminder to check it off in our binder. Then I give the bookmark
back, they cut off the top portion that have my signature and their name
and they put that in the raffle box for weekly prizes.

I tell them to keep the bottom portion if they want earn a Bingo card. 2
bottom portion bookmarks will earn a bingo card for the next month. Every
Wed. we announce 5 numbers. Winner of bingo cards get bigger prizes.
