[ISLMA-Share] Library Sign in Procedures

We have two Chromebooks on the front counter. We have a Google Chrome form
they fill in. (Just first and last name.) It automatically dumps into a
spreadsheet that time and date stamps it. I email out the link to this form
to all staff so they can check in and see if their minions made it down.
