[ISLMA-Share] Reading interest survey

[Email list for the Illinois School Library Media Association]

Would anyone happen to have a survey to share that could help reluctant readers find books they may find interesting. I'm thinking something along the lines of assigning numbers or letters to things they find interesting (i.e., specific sports, being scared, puzzles, etc) then offering genre suggestions based on their "score". Anything like this already available?
