[ISLMA-Share] lost books

I have tried forgiving lost books several times and have had little success. 

What grade level is the student?
What happened to the first book? Lost or damaged 

What will they do differently next time? They have to write me a letter saying what will be different. 
I put a note on patron in destiny when I forgive a fine. 
I have also tried reading off a fine -that took a great deal of my time.
