[ISLMA-Share] End of the Year Survey for Staff and Students

Hi!

I've seen a few emails about this, but there wasn't much of a response so I
figured I'd give it a go :) This will be my first year giving an end of the
year survey for staff and students. I provide computer classes to students
in K - 4, library classes to students in K - 8, and I oversee 3 libraries
in 3 buildings by myself. Does anyone have any examples or ideas to share
as far as what kind of questions you asked - especially for the staff one?
