[ISLMA-Share] Conference Evaluations for PD credit due by December 1

Just a friendly reminder - In order to receive PD credit for the conference, you need to submit an online evaluation<https://docs.google.com/forms/d/e/1FAIpQLScSOet-c1YNRkGlI21CsGYVaZAZlOd4bjNePQbcmRRM4M57XA/viewform> for each session you attended.
Evaluations must be submitted by DECEMBER 1, 2017.  Evidence of Completion forms will be emailed to you within 10 days of submission.

All the information for getting credit is listed on page 14 of the conference brochure, or see the attached pdf.
