[AISLE-Share] YA checkout form?

I would send a waver at the beginning of the year and had parents sign it stating they would talk to their student about reading material they felt was appropriate for them. Then I put a block on their Polaris account if they were not allowed to check out YA material. A little bit of a process but it protected us we felt.
