[AISLE-Share] YA checkout form?

We are in a smaller school. I usually handle this on a case by case basis.
If the book is questionable for the student requesting it I print out a
description of the content and its recommended age levels. I have the
student take that sheet home and get a parent's signature. My condition for
other "older" books in the elementary library is for the student to be at
least in 5th grade. The type of YA materials we have in our collection
usually linger around the Hunger Games and Maximum Ride status as far as
"mature" content is concerned.
