blockquote, div.yahoo_quoted { margin-left: 0 !important; border-left:1px #715FFA solid !important; padding-left:1ex !important; background-color:white !important; } Google forms was great because it scores them.  I did have a non-graded question for their name or you just get their ID numbers.  I would offer  paper also next time.  Our readership declined because some ELA teachers told students they could not read other books while they read their class novel.  Nice way to discourage reading!  Those classes did not visit the library for months.  The circulation on the Caudills still has been high over the entire year even after the program ended.  The books have been great.
My thought is that if the program ran through April more students may have participated.  I think the program should run on a school year schedule.  I usually can't buy the books until next years budget and students would have the same teacher all year for consistency.  I do not book talk the books until the new school year.

We do our responses on a Word Press blog.  Students do their responses as comments and I keep track of them on a Google Sheet when I approve their posts.  We have been doing it this way for 10 years.  I have seen a huge decline in Caudill responses the past two years, but I have seen a large increase in high school responses for Read for a Lifetime during the same time.  (I'm not sure what has brought down the Caudill so much recently but the lack of participation follows the retirement and non-replacement of the certified elementary librarian.)
Here are the links to see what they look like:  https://www.gcsdstaff.org/HS_Library/https://www.gcsdstaff.org/MS_Library/
One of the bonuses to the electronic form is that kids can work on them all summer since I set them up right away in the spring.

Yes, because we do a voting party. https://pbljh2019caudillnominees.wordpress.com/But response forms were down this year, and I know many kids are reading the books. I think a simpler form and in paper plus online would be a good step, plus more time to fill them out in the library.

Yes. I just put the cover image in for each book for voting. It wouldn't be a good tracker. . ..Google sheets?
The paper bookmarks are still popular.
