[aisle] form for recording patron purchase requests

I have used a "suggestion box" for several years. It works great. You can
see it on the Elm School library catalog.
<https://d181.booksys.net/opac/elms/index.html#menuVisual> There have been
458 suggestions since I started in 2014, 62 this year. I make tabs in the
spreadsheet for each year. I get notified when someone makes a suggestion.

I give the disclaimer that I read reviews of all the books suggested and
not every suggestion will make it into the library, but the vast majority
do. Kids are thrilled when a book they suggested becomes part of the
library. They've made some great ones.

The information goes into a spreadsheet. I make notes by each suggestion
indicating when I bought it or why not. Just a few words.

I got this idea from the AISLE President, Mary Morgan Ryan. It works so
well!
