To all others: As of today, I've sent out all Evidence of Completion for
anyone who filled out an evaluation AND both signed in and out. If you
believe there has been a mistake, please email me at
licenserenewal at aisled.org. DO NOT respond to this email directly.

Evaluation forms will be open until November 15, 2019.
