[aisle] 5 Essentials Survey Questions

The library is not a part of this.  The asst. principal in charge of
curriculum is in charge.  On a given day, all study halls rotate through a
computer lab to take the survey with him. He follows up with absences later
on in study hall.  Anyone without a study hall is then pulled out of a
class.
