[aisle] google voting form Monarch and Bluestem

Hello all!  I have paid my fees for both the Monarch and Bluestem
resources.  There is not a google form for voting purposes on the
membership sites.  Before I create my own google forms, has someone already
create these with the pictures of the book covers?  I would LOVE, LOVE,
LOVE for someone to share or post on the paid sites for bluestem and
monarch, please!  If I end up creating it myself, I will gladly share and
pass on for anyone to use.  Thank you in advance!
