[aisle] Electronic device parent permission

Hello,

In the past we had parents and students sign a permission slip for use of
Kindles and Playways. Essentially acknowledging the responsibility for
proper care and handling, and returning it or being charged a replacement
fee.

We are looking to update this. Do any of you have a similar form you would
be willing to share? Or does your district's technology use form cover this?
