[aisle] Interest Checklists for Student Checkout (K-2)

I used Google Forms to create a book menu.  I've included the link to a
sample for you to look at.  I have a menu for PK-1, 2-3, and 4-5.  I change
the options every few weeks.  The 2/3 is a little different in that for
fiction I have the topics and checkboxes for picture books or chapter books
(when available) so they can let me know which they prefer, but don't have
pictures for those topics.
