[aisle] Looking for examples of challenge policies

Here is a copy of a form I developed years ago.

The more specific information that you require the person to complete on
your Challenge Form the  more likely they will think through their
challenge.

This should also be accompanied or included in your Selection Policy which
should discuss the Professional Review sources that you use in selecting
your materials.
