[aisle] Creative Ways to Keep Track of Award books read?

Hello,
I'm in an 8th grade center. I have a Google Form for students to complete
after reading a book from our Reading Challenge list (appropriate Caudill
and Lincoln books). Items on the form:
Name, ELA teacher, Book title
3-4 sentences in your own words what the book is about
What part of this book was the most: engaging, interesting, funny, sad,
suspenseful OR emotional?  (You select on of the ideas!) Explain your
answer.
On a scale of 1 (poor) to 5 (excellent) how would you rate this book & why?

I make it a competition among ELA teachers and have a sign for each teacher
at the entrance of the library to show the number of books read by that
teacher's students.

I provided little laminated book covers for the ELA teachers to display in
their room as well.  When a student reads a book from the list I have a
little slip with the student name that the teacher posts next to the book
cover.
