[aisle] opt-out policies

For those of you who allow interlibrary loan for your students which might give them access to more mature titles, do you have an opt-out policy of any type, where parents are advised at the beginning of the year that this is a possibility and if they want to restrict their students access, they have to sign something and submit it?

Or do you do the opposite and only allow those with permission to check out books that fall above their grade level?

I'm trying to get an idea of what libraries around Illinois are doing. Thanks for any input you can give, and if you have a policy you'd be willing to share, I'd love to see it.
