[aisle] End of the Year Survey Help

Good afternoon,

I am finishing up my first year within a new district.  At my previous
district, I had over twenty years of experience collaborating with various
departments and could anticipate their needs.  I have struggled a bit this
year.  The library was completely "off-limits" last year as it was
repurposed as a large classroom, so enticing teachers and students to come
back into the space has been a bit challenging.  While I've had success
with most of the ELA department, I would love to do more collaborating
across grade levels and content areas.

Since I'm new, I would like to send out a survey to help me plan
programming and lessons for next year.  Have any of you surveyed your staff
to determine their and students' needs?  If so, do you mind sharing so I
can "borrow" phrasing and content?
