[aisle] accountability for award books

I do a Google form with student name and three questions about the book. They aren’t meant to be trick questions, just in depth enough to show they read the book. They get a raffle ticket for each survey they complete.

I would really like to push both the bluestem and Caudill in my k-8 building next year but I would like to have students hand something in for accountability. Do you use anything that has worked well?
