We use a GoogleForm and keep it pretty simple. (Like, we don't want it to
feel like homework, but a little bit of accountability is needed.)

   - Name
   - Grade
   - English/Reading Teacher
   - Book Read (they just check the bubble next to the cover image)
   - Then we have an open-ended question as our "proof" that reads thusly:

"Please briefly share some of your thoughts on the book in the space below!
We don't need a book report (unless you really want to write one!). Just a
couple sentences will do.
For example, something you could write about here might be why you did or
didn't like the protagonist, or whether or not you liked the ending and
why."

When I have a chance to present to a class about the program, I always note
that if all they write on the form is "I liked it" or "It stank," then
we're probably going to send a follow-up email ("What did you like about
it?" or "Why do you think it stank so much?"), but that they don't have to
write a full-on essay or anything. (Though I've also had some students who
really took the opportunity to craft lovely paragraph-length reviews on the
form, which the other staff and I love to read.)

What's nice about using a GoogleForm is that it auto-populates a
spreadsheet with their responses, which is really nice to have for later
reference, especially when recommending titles I haven't read yet to other
students. We do still give students the option of verbally checking in
in-person, too, and some students even like to do both.
