[AISLE-Share] YA checkout form?

I don't have a form but I do have this on my website and I tell them this @
orientation and if I remember when they check out a YA.... '*If a book ever
makes you feel uncomfortable, bring it back and check out something else.
What is not a good fit for you, might be a good fit for somebody else or
you in the future.  Library books are optional free reading, not a
requirement."*
